Information about Registration
Please bring a valid driver's license or photo identification card, your current insurance card or cards and your physician orders.
You will be asked to pay any co-pays as well as your deductible.
Non-Emergency patients - Non-Insured patients will be asked to make financial arrangements proir to the day of their procedure.
All procedures deemed elective by Colquitt Regional Medical Center must be paid in full prior to the procedure.
